Jacque's Dance Expressions has implemented a policy to give you the necessary information you will need when registering your dancer for classes. Please read each paragraph carefully and initial in the spaces provided stating that you understand and agree to the information provided. If you have any questions regarding any of the following information please let me know.

Billing and late fee-

Jacque's Dance Expressions will mail statements to the address that is on file, any address changes will need to be submitted in written form. Statements are mailed to the parent/guardian as a courtesy. It is the responsibility of the parent/guardian to know that tuition is due on the first (1st) of every month and is not considered late until the fifteenth (15) of the month. There is a late fee of $15.00 applied to any account that has a remaining balance on the sixteenth (16) of the month. If you must pay tuition by splitting up the balance, be sure to have the balance paid by the fifteenth (15). If you will be mailing your tuition payment to Jacque's Dance Expressions, it will need to be post marked on or before the fifteenth (15). Again, a $15.00 late fee will be applied to every account that has a balance over $0.00 on the sixteenth (16) of every month.

If an account becomes more than 60 days past due we will no longer allow the dancer or dancers to continue taking classes until this balance is paid in full.

Pro-rating Tuition

Jacque's Dance Expressions will pro-rate tuition for absences only when there is a two-week notice given. There is the option of taking make-up classes. However, dancers need to be aware that when it is close to recital, every class is working on dance routines and it is difficult to make up classes. Classes will not be pro-rated for any holiday break that the studio is closed.

If a new student begins dance lessons during the middle of the month, the classes that were missed prior to the start date will be pro-rated. Again, during any holiday vacations that the studio is closed, tuition will NOT be pro-rated.

Discontinuing dance

If your dancer chooses to drop dance lessons at Jacque's Dance Expressions, a two-week written notice is required. You will be charged for classes up to the date that notice was given and for the two weeks following whether or not your child attends class during that time. The two-week notice will only be bypassed in the event of an injury accompanied by a doctor’s note.

Dropping off students for dance

Please be sure that Jacque's Dance Expressions has all emergency information on file for the student that you will be dropping off. Children under the age of 18 will not be permitted to leave the studio without adult supervision. If you would like your child to be able to leave the studio please submit written permission stating as such. Please talk to your children about the importance of remaining inside the studio until someone has arrived to pick them up. The well being of our students is not something that we are willing to compromise. Thank you for your cooperation.

Returned checks-

Jacque's Dance Expressions will apply a $25.00 returned check fee to your account for any checks returned to us for any reason. A letter will be sent making you aware of the returned check. Please be sure the balance is paid as soon as possible. A balance due could result in a late fee.

Registration

A completed registration form is required prior to the student beginning class. If any information given on the registration form at the time of the initial registration changes, a new registration form will need to be completed. It is important that all the information on file is complete and accurate.  Jacque's Dance Expressions has re-registration on an annual basis. Since we operate on an academic year, re-registration is every September. At that time all existing students will need to complete a new registration form with current information. There will be a registration fee applied to all accounts with students beginning prior to June 1st of that year. Only students that have registered after June 1st will be excused from the re-registration fee.

Recital and Costumes

Jacque's Dance Expressions will hold an annual Recital in which all dancers will perform. There will be costumes ordered for each individual student, for each dance (i.e., Tap, Jazz, Ballet or Lyrical) they will be performing in the Recital. There will be a charge for the costume(s) that can range anywhere from $40.00 - $85.00 per costume. If your child will be performing in the annual recital, you will be responsible for paying (in advance) for the cost of the costume(s). Costumes will not be given out unless they are paid for. If at the time that we begin taking measurements for costumes, you decide that you do not wish your child to participate in the recital, it is your responsibility to inform us so that a costume is not ordered. If a costume is ordered for your child and for some reason they will be unable to participate in the recital, there will be a charge for canceling the costume order from the company. The companies sometimes put deadlines on canceling costumes, if your decision not to participate falls after the deadline for canceling the costume, you will be responsible for the full amount of the costume.

There will also be a "Recital Fee" applied to all student accounts that are participating in the recital. This fee is for the costs that are incurred during the preparation of the recital. It is very important that the recital fees are paid, as this is how the recital is paid for.

Class Cancellation

It is important to know that all class times and days are subject to change in order to accommodate new students. Classes are subject to change and/or cancellation if there are fewer than six (6) students enrolled in the class.

Jacque's Dance Expressions is not responsible for lost or stolen items. Please be sure that your child’s name is on all personal items. (Dance shoes are an important thing to have names on.)

WAIVER

In consideration of the acceptance of my registration, I the undersigned am legally bound for myself, my heirs, and successors and do hereby waive and release Jacque’s Dance Expressions, its sponsors, host venues and all persons and agencies connected with this studio from all claims for damages arising from my participation in and travel to and from this studio or studio events, including without limitation, personal injury. I certify that the above named "Student" is in good health, physically fit and capable of participating in classes. I grant permission to Jacque’s Dance Expressions to use the "Students" name and likeness for any purpose. I have read and understand the policies set forth by Jacque’s Dance Expressions.
